It's the unexpected offload. If AWJ had gone down, Itoje supports, by offloading (especially that late, after the pattern has been set up) he disrupts the structure. Worth doing, but it's the risk. Half breaks are easily the most turned over attacking play.

There may have been, or at the very least, aware of the necessity. Once it slowed down, everyone would have been getting ready for the next phase.
